ADULTS

General Public Safeguarding Referral Form
This form should be completed by people who wish to report an incident of abuse or when abuse is suspected.

Please email or fax the form to the Initial Assessment Team, contact details below. This is in line with Safeguarding Adults in Trafford Policy and Procedure.

If you suspect someone is being abused and is in immediate danger you should ring the emergency services on 999.

TARGet (Trafford Adults at Risk Group)

This is the multi-agency group, led by Greater Manchester Police, supported by NHS agencies in Trafford, Trafford Council and the Probation Service. It has been set up to reach out to Trafford Adults at Risk who fall outside of the usual "safeguarding" categories.  These might be vulnerable adults, of any age, who you know of living in your local community (if you are a resident) or identified through your practice (if you are a professional). These vulnerable people may be experiencing low level harm, harassment, name calling or physical harm at the hands of other people. This low level harm can quickly escalate to become more serious and potentially dangerous. This is what we aim to stop, through careful, targeted action.

Alternatively, these may be adults who themselves are vulnerable. They may be users of substances, alcohol or be mentally and/or physically unwell. They may also be using health services, including mental health crisis services, the emergency department or GP services frequently. They may make frequent contacts with the police service or are at risk from "Mate Crimes" or other forms of abuse and exploitation by unscrupulous people in their local communities. Through careful, targeted action we want to reach out to offer support to these very vulnerable people. To do this, we need your help.

If there is someone you are concerned about, then please use the new, unified Safeguarding Adults Referral Form and make a referral. Please indicate in the space provided that this is a TARGet Referral. You must have permission from the person to refer them to TARGet.
