New Patient Registration
If you live within our practice boundary (see map below) you are welcome to register with us and our reception staff will be happy to guide you through the procedure.
You will need to complete a registration form (GMS1) along with a completed new patient questionnaire New Patient Questionnaire (Adult) or New Patient Questionniare (Child) this can be downloaded and printed and sent back to us via email. During the pandemic we are not requiring you to come into the practice to register. You would normally need to come into the surgery to confirm your eligibility for registration which can be quickly confirmed from your address - you would need to bring with you proof of address i.e. recent utility bill. We also require one form of photographic ID. Please note that whilst we are currently in lockdown as a result of Covid-19, you can email your forms into the practice along with ID. Please email your forms to firstname.lastname@example.org.
When completing your GMS1 form in order to avoid delay with your registration, please sign on the line indicated below:
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Medical treatment is available from the date of registration.
